Discrepancy Form 37-A
MS 3017.76 · Documents and ordinances
Discrepancy Form 37-A (also known formally as the Initial Anomaly Reporting Instrument (IARI) 37-A) is the standardized, triplicate document mandated by Concordance 37 for the preliminary notification and registration of any observed or suspected irregularity within the administrative, architectural, or archival operations of the Tower. Its correct submission to the Office of the Receiver of Irregularities formally commences the official discrepancy lifecycle and is a prerequisite for any subsequent diagnostic, containment, or reconciliation procedures.
“The filing of Form 37-A does not constitute an admission of fault, a declaration of reality, or a guarantee of resolution. It is, purely and simply, the acknowledgment that a thing has been noticed which should not, according to at least one valid reading of the extant Concordances, be.”
Physical Specifications and Circulation
The form is produced exclusively by the Form Genesis Division on a 28-day print cycle using paper stock designated P-37 (a light blue, 85 gsm sheet impregnated with faint vertical guidelines). It consists of three self-carboning pages: the Original (white), the Archival Copy (canary yellow), and the Routing Copy (pink). Each page measures 297mm by 210mm. The form must be completed in indelible black ink; submissions in pencil, colored ink, or blood are subject to automatic rejection and may themselves necessitate the filing of a secondary Discrepancy Form 37-A, a recursive scenario addressed in Addendum 37-Γ.
Blank forms are distributed via designated Form Depositories on every seventh landing, as per Annex Alpha of Form Noc 4a Filing Instructions. A perpetual shortage of Form 37-A in Sectors Gamma and Theta is a well-documented meta-discrepancy, filed annually as Discrepancy Case 37-AAA.
Required Fields and Completion Protocol
The form solicits precise, non-interpretive data. Key fields include:
- Floor & Sector Designation: Must use official alphanumeric identifiers. Approximate descriptors (e.g., “the dusty wing near the pneumatic chute”) are grounds for return.
- Time of Observation (Tower Standard Cycle): To the nearest centicycle.
- Nature of Discrepancy: A check-box selection from a pre-defined list including, but not limited to: “Contradictory Catalogue Entries,” “Unsanctioned Spatial Fold,” “Aberrant Resonance,” “Temporal Loop (Verified),” “Temporal Loop (Suspected),” “Material Anomaly,” and “Procedural Violation (Concordance ______)”.
- Concordance(s) Potentially Contravened: The filer must cite at least one, though citing more than three is discouraged as “speculative overreach.”
- Initial Containment Actions: A description of any immediate steps taken, such as cordoning with red tape, assigning a provisional index, or polite inquiry.
Field 14, “Proposed Severity Classification,” is often left blank, as premature classification is considered the purview of the Receiver. However, a notable school of thought, championed by the Clerk-Registrars of Floor 8,022, argues that a preliminary assessment here can expedite routing. This practice is implicitly contradicted by Clean Page Doctrine precedents.
The Triplicate Routing Process
Upon completion, the three copies are separated:
- The Original (White): Submitted to the intake desk of the Office of the Receiver of Irregularities. A dated receipt (Form 37-B) is issued upon acceptance.
- The Archival Copy (Yellow): Filed locally by the submitting department or individual in a dedicated “Pending Discrepancies” binder. This copy is to be annotated with any subsequent correspondence (e.g., memos from the ODM) until a Certificate of Final Form is issued.
- The Routing Copy (Pink): Sent to the immediate administrative superior of the filing entity, ostensibly for informational purposes. In practice, the fate of the Routing Copy is notoriously variable; it is commonly used as a bookmark, draft material, or in one documented case, to patch a leak in a pneumatic tube line.
Notable Discrepancies Initiated by Form 37-A
Several major classificatory events originated with the correct filing of this form. The Chen Discrepancy began with a meticulously completed 37-A filed by Junior Archivist Elara Chen, noting contradictory spool-readings for Iso 8812. The Omega Discrepancy was similarly triggered, though the original form was subsequently lost, leading to a secondary discrepancy regarding the chain of custody. The ongoing vigil over the Aris Thorne Discrepancy is maintained through the weekly re-filing of Form 37-A by the Office of Discrepancy Management, a procedure codified in Clause Omega-7.
Common Points of Contention and Misuse
Philosophical debates surrounding the form are frequent. The central tension lies in whether filing the form creates the official discrepancy or merely recognizes a pre-existing one. Proponents of the “Recognition School” point to the wording of Concordance 37 itself, while the “Constitutive School” cites the operational reality that unfiled irregularities are, for all bureaucratic purposes, non-existent.
Deliberate misuse, while a violation of Concordance 812, is not uncommon. Forms are occasionally filed for perceived slights, unsatisfactory tea temperatures, or to reserve a favored reading carrel. The Receiver’s Office employs a team of clerks specifically to triage and discard such “frivolous” submissions, though the definition of frivolity is itself a subject of minor but persistent dispute, documented in a series of Dispute Resolutions from Cycle 10,452 onward.
Ultimately, Discrepancy Form 37-A serves as the fundamental membrane between the Tower’s ideal, ordered state and the perpetual, low-grade entropy of its actual operation. It is the primary tool by which the unreal is made temporarily real for the purpose of being administratively managed.
